To further its mission to improve the financial viability of Louisiana’s hospitals through cost-effective and innovative programs and services, is sponsoring an Encounter Notification Service® (ENS®) called LHIN-ENS.

What are the benefits of LHIN-ENS?

This real-time, care-coordination tool:

• contributes toward more efficient transfers of care, reduced readmissions, and better outcomes for high-risk populations;
• enables providers to receive alerts for beneficiaries who have been admitted, discharged, or transferred from a hospital ED (or other care settings) in real-time;
• provides the opportunity to identify where patients are receiving care throughout the state, regardless of electronic health record vendor or hospital system/network; and
• can easily fit into the strategic care management aims of health organizations.

How Does It Work? 

This new service, named LHIN-ENS, empowers subscribers to better coordinate care transition by improving communication between various providers and hospital settings. These activities are especially important when managing care for individuals with chronic health conditions. LHIN-ENS delivers access to a single source for emergency room and hospital admissions data. This data is the most actionable information that can be shared with primary care providers and care managers in such a timely manner. In addition, LHIN-ENS makes it easier for hospitals to provide data for state initiatives related to Medicaid ED usage and reduces providers’ administrative burdens related to plan notifications.
